Greater Chicago Home Health, a Medicare certified, CHAP accredited agency, is a new partnership between Norwegian American Hospital and ASI Homecare Services. GCHH is committed to improving the health and wellness of our clients through the provision of high quality, cost effective health services.

We are seeking per diem registered nurses to join our rapidly expanding operation. Registered nurses will manage the care of patients in their home by completing in-person health assessments and coordinating care, including medication management, disease management, wound management and keeping members healthy and independent.

Greater Chicago provides health services in Cook, Lake and DuPage counties. Home health offers the perfect opportunity to provide one on one care to address the clinical needs of your patients along with a highly flexible schedule.
Requirements:
• Current and valid Illinois license to practice as a Registered Nurse
• Minimum (2) years medical and surgical nursing experience required
• (2) years Home Health experience required
• Skilled in electronic charting and email
• Strong communication and interpersonal skills required Highly competitive pay.
